What is a Passport Clinic?
Since it is not always possible for people to come into one of my offices to get assistance with their passport applications, I’m bringing my office to you. 
At the clinic, you can receive assistance filling out the application, have your pictures taken, and have applications reviewed to ensure all materials and documentation are included and is ready to take to your guarantor for signing.

What Should You Bring to the Clinic?  
  • Proof of Citizenship - Canadian Birth Certificate, or Citizenship Card/Certificate (Original must be sent with application)
  • Additional Document to Support Identity - Government-issued ID with signature (e.g. Driver’s Licence, Health card, Old Age Security Card, S.I.N. ) Only a photocopy of both sides of this secondary document is required.  We will have a photocopier at the clinic for this purpose.
  • Contact information for two references (non-relatives who have known you for at least 2 years) and One Emergency Contact.  References do not need to sign your application.
  • Current Passport (if you have one)

If you’re getting your first passport, or renewing a passport that is more than one year past expiry, you will need a Guarantor - a person (can be a family member), who has known you for at least two years and who holds a valid adult Canadian Passport). 
 

The Guarantor is required to:
  • Sign the back of the stamped passport photo
  • Complete Section 2 of the Application
  • Sign any photocopies of the Documents in Support of Identity

If your Guarantor can attend the Clinic with you, you’ll be able to leave your completed application at the Clinic.  If not, you may complete and have checked, all remaining requirements (other than the Guarantor) of the passport application, gather your Guarantor signatures afterward, and return the application to our office for shipping to Ottawa.
 
A photographer will be on-site to take pictures for $20 (Cash and Credit Card only)
 
Fee’s for Passports
- $87 for Adults, and $22-$37 for Children. Payment must accompany the application either by credit card, certified cheque or money order. 
 
Certified cheques or money orders must be made out to the Receiver General for Canada
